General program guidelines include:


  • At least two enrolled individuals, including one qualifying payroll employee.
  • Employer contributes at least 50% of the employee's premium.
  • Employee eligibility may be set between 17.5 and 40 hours per week.
  • Group must meet applicable membership and carrier requirements.
